This is the Registration Form you will need to complete and return along with your payment in order to register to attend the PTHS MEGA REUNION.
There are two ways to use this form. You may fill this form out online then print it out and mail it back along with your payment or you may print it out first, fill it out and send it back along with your payment.
Please make all checks payable to "PTHS
CLASS OF 75".
You may pay with check or money order.
